You may download the program information sheet (a Microsoft Word document you type directly on) by clicking here. Once you have completed the program information page, please save it and send it as an e-mail attachment to Jason Eslinger at jasoneslinger@ighsau.org. Team photos, along with a photo ID, should also be e-mailed at the same e-mail address. Please do not send photos larger than 5 MB in size.
